Does Insurance Cover In-Home Physiotherapy in Ontario? A Guide to Coverage and Direct Billing

If you are thinking about in-home physiotherapy in Ontario, one of the first questions is usually about cost. Will insurance cover it? Can the clinic bill your insurer directly? The answer depends on your coverage, and understanding the main funding options can save you time and money.

Does OHIP Cover Physiotherapy?

OHIP covers physiotherapy only in limited situations. Publicly funded physiotherapy is generally available for people who:

  • Are 65 or older, or 19 and under
  • Are receiving physiotherapy after a hospital stay that required an overnight admission, for certain conditions
  • Are eligible for services through Ontario Health atHome (formerly Home and Community Care), for example due to a medical need that makes it difficult to leave the house

Eligibility rules and available visits can change, and publicly funded programs may have waitlists or limits on the number of treatments. Confirm current details with your health care provider or the Government of Ontario.

Private Extended Health Benefits

For many Ontarians, private insurance is the main way physiotherapy is paid for. Most employer group plans, and many individual plans, include paramedical coverage for physiotherapy. Things to check in your plan:

  • Annual maximum: the total dollar amount available per year, sometimes shared with other services such as chiropractic or massage
  • Per-visit limit: a cap on what is reimbursed for each treatment
  • Percentage covered: many plans reimburse 80% or a similar share
  • Referral requirement: some plans ask for a doctor's referral, though many do not
  • In-home coverage: most plans cover physiotherapy based on the service, regardless of location, but it is worth confirming

What Is Direct Billing?

Direct billing means the clinic sends the claim to your insurance provider for you. You pay only your portion, such as a co-pay or anything above your coverage, rather than paying the full fee upfront and waiting to be reimbursed. Direct billing is available with many, but not all, insurers and plans.

To make the process smooth:

  1. Have your insurance provider name, policy number, and member ID ready when you book.
  2. Ask your insurer how much of your annual maximum remains.
  3. Confirm whether the clinic can direct bill your plan before your first visit.

Other Ways Physiotherapy May Be Covered

  • WSIB: if your injury is work-related and your claim is approved, physiotherapy may be covered through the Workplace Safety and Insurance Board.
  • Auto insurance: if you were injured in a motor vehicle accident, treatment may be covered under your accident benefits.
  • Veterans and federal programs: eligible veterans, RCMP members, and some First Nations and Inuit clients may have coverage through federal programs.
  • Health spending accounts: many employers offer flexible spending accounts that can be used for physiotherapy.

Tax Benefits

If you pay for physiotherapy out of pocket, the cost may qualify as an eligible medical expense on your income tax return. Keep your receipts, and speak with a tax professional about what applies to you.

Questions to Ask Before You Book

  • Does my plan cover physiotherapy, and how much is left this year?
  • Do I need a referral?
  • Can the clinic direct bill my insurer?
  • Is there any out-of-pocket cost for me?
  • Does my plan cover in-home visits?
