Whiplash

Physiotherapy for Whiplash

Whiplash is one of the most common neck injuries, and one of the most misunderstood. Because the symptoms often don't show up until a day or two after the event, many people assume they've escaped an accident unharmed, only to wake up stiff, sore, and struggling to turn their head. The encouraging news is that with the right care started early, most people recover well.

What Is Whiplash?

Whiplash happens when the head is suddenly and forcefully thrown backward and then forward, stretching the muscles, ligaments, and joints of the neck beyond their normal range. The rapid whip-like motion is where the injury gets its name. It's essentially a soft tissue injury of the neck, and like other sprains and strains, the damage is to the tissues that support and move the joint rather than to the bones themselves. It falls under the broader umbrella of neck pain, but has its own distinct pattern and causes.

Common Symptoms

Whiplash symptoms often develop gradually over the 24 to 72 hours after the injury. The most common are neck pain and stiffness, reduced range of motion, and headaches that typically start at the base of the skull. Many people also experience tenderness across the shoulders and upper back, and sometimes referred shoulder pain. Some people notice dizziness or a feeling of unsteadiness, which can overlap with dizziness and, in some cases, positional vertigo. If the force irritates a nerve, you might feel tingling or numbness travelling into the arm, a sign of a possible pinched nerve that's worth having assessed.

What Causes Whiplash?

Car accidents, particularly rear-end collisions, are by far the most common cause, since the impact forces the head to snap back and forth in an instant. But whiplash isn't limited to the road. It can also happen from sports contact and tackles, a slip or fall, a blow to the head, or any sudden jolt that throws the head rapidly in one direction and back. The amount of force involved doesn't always match the severity of symptoms, which is why even a low-speed collision can leave someone with a genuinely painful injury.

How Physiotherapy Helps

Physiotherapy is one of the most effective treatments for whiplash, and starting early makes a real difference to how quickly and completely you recover. For a long time the standard advice was rest and a soft collar, but we now know that gentle, guided movement leads to better outcomes than immobilizing the neck.

A physiotherapist will assess your range of motion, strength, and any nerve involvement, then build a plan around your stage of healing. Treatment usually includes gentle range-of-motion work and stretches to restore movement without aggravating the injury, hands-on therapy to ease stiffness in the neck and upper back, posture and shoulder work such as shoulder glides, and progressive strengthening as symptoms settle. Because whiplash so often follows a car accident, we cover the full stage-by-stage recovery path in our guide to rehab after a motor vehicle accident. Getting on it early is also the surest way to avoid the lingering pattern described in preventing chronic pain.

The Role of Massage Therapy

Registered massage therapy is a valuable complement to physiotherapy for whiplash. After the injury, the muscles of the neck and upper back often tighten and guard protectively, which adds to the pain and stiffness. Targeted soft tissue work eases that muscle tension, improves circulation to the healing tissues, and helps restore comfortable movement. If neck tightness is a recurring problem for you, our article on recurrent neck pain has more.

When to Seek Care

Because whiplash symptoms can be delayed, it's worth getting assessed soon after any accident or jolt to the neck, even if you feel mostly fine at first. Early treatment is consistently linked to faster recovery and a lower chance of symptoms becoming long-term. Seek prompt medical attention if you have severe pain, weakness, numbness spreading into the arms, or any signs of a more serious injury.

One helpful note: many extended health plans and auto insurance policies cover physiotherapy and massage therapy after a motor vehicle accident, and we can help you navigate that process.
